He was predeceased by: his parents, Jesse Polk and Elese Berkeley Polk; and his siblings, Jesse Polk Jr. of Dewey Polk, J. B, Hayward Polk, Clyde Polk, Ruby Smith and Opal Proctor.

He is survived by: his wife Clara Mae Polk; and his sisters, Ethal Proctor of Dallas and Mary Joe Polk of Dallas. He is also survived by nephews, nieces, and a broad community of relatives and friends.
