He is survived by: his wife Betty Polak; his daughters, Darlene Lockard (Gary), Tammy Morgan (Ken) and Diane Brown (Matt); his grandchildren, Courtney Kolski (Chris), Lauren Krogman, Brian Morgan (Alexandra), Houston Hemphill and Mackenzy Hemphill; his great grandchildren, Mason Kolski, Liam Kolski and Riley Kolski; his stepgrandchildren, Blake Brown and Stephanie Brown; his siblings, Rudy Polak (Mary Ann), Dennis Polak (Brenda) and Jo Ann Priesler; his sister-in-law Darline Polak; and his brothers, Johnny Polak (Diana) and Raymond Polak (Janice). He is also survived by his family, friends.
