He was predeceased by: his parents, Johnny W. Lewis and Betty Jo Lewis. He is survived by: his siblings, Cheryl Hays (Jim), Jerry Lewis (Ronda) and Debra Parks (Scott); his nephews, Jason Hays (Amanda), Jeffrey Lewis (Carrie), Justin Hays (Kristin), Jeremy Lewis (Kendyll) and Bryan Parks (Kelly); his niece Ashley Parks (Zoey); and his great-nieces and great-nephews, Jake Lewis, Jaxen Lewis, James Lewis, Jordan Lewis, Malikai Parks, Wyatt Hays, Walker Hays, Jayden Hays, Juliette Hays, Charlotte Lewis, Aspen Parks and Khole Parks.
