He was predeceased by: his parents, Edward George Slavik and Viola Rose Worrad Slavik; and his sister June Slavik. He is survived by: his wife Barbara Slavik; his children, Lisa Campbell (Lee), Michael Keen-Lynch (Billy) and Randy Helmcamp (Jessica); his siblings, Donald Slavik (Maureen), Ray Slavik (Patricia) and Gail Rotherforth (Darryl); and his grandchildren, Annabelle, Abagail, Andrew, Alice Helmcamp, Gavin Lynch, Jakhob Cangemi, Leela and Leeland Campbell.
