We are sad to announce that on May 20, 2019, at the age of 80, Samuel D. Axelrad M. D. (Charlotte, Texas), born in Houston, Texas passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Jack Harry Axelrad and Sylvia Axelrad; and his brother Dr. Moise Axelrad. He is survived by: his wife Charlotte Axelrad; his son Chris Axelrad; his daughter-in-law Sandra; his granddaughters, Claudia Haney and Maria Axelrad; his son Dr. Larry Baum; his daughter-in-law Cuqui; his grandchildren, Owen and Ava; his daughter Ann Axelrad Goldfarb; his son-in-law Dr. David Goldfarb; his grandsons, Max, Will and Gus Goldfarb; his brother Dr. David Axelrad; his sister-in-law Carolyn; his sister Sandra Axelrad Bocarra; and also Laurent. He is also survived by cousins, nieces, nephews, grand-nieces and grand-nephews.
