He is survived by: his wife Josie Velazquez (Gaitan); his children, Guillermo Velazquez, Juan Rodriguez, Jr., Julisa Velazquez and Zariah Rodriguez; his siblings, Felipe Velazquez, Adrian Velazquez, Martha Velazquez, Violeta Velazquez, Guillermina Velazquez and Veronica Velazquez; and his grandchildren, Anthony Gaitan Jr., Aaliyah Gaitan and Baby Al. He is also survived by niece and nephews.
