She was predeceased by: her parents, William Proctor and Louise Betts. She is survived by: her husband Samuel Polk; and her children, Curtis Betts, Shawnette Martin, Otis Betts, Latonya Betts, Olivia Polk and Samuel "Fathead" Polk. She is also survived by grandchildren; and great-grandchildren.
