He was loved and cherished by many people including: his wife Pat Cullison; his children, Carlene Morrison (Monty) of Burkburnett, Gail Hawk (Tom) of Silsbee, Teri Miller (John) of Mesquite, Janet Duvall (Duncan) of Tulsa, Oklahoma and Chris McElwain (Nicole) of Florida; his grandchildren, Heath Holtzen (Tina), Rusty Holtzen (Jessica), Russell Miller (BJ), Karla Witrey (Ben), Daniel Hawk (Dawn), Jo Lynn Hawk, Scott Hawk (Amber), Jeni Ann Massey (Zach), Ray Hawk (Maggie), Drew Duvall, Chad McElwain (Shakerah), Amie Stepp (Mike) and Capri McElwain; and his great grandchildren, Charessa Munson (Brian), Alexandra Holtzen, Baylor Holtzen, Amber Miller, Emily Witrey, Trinity Berry, Maria Cangemi, Virginia Hawk, Angel Hawk, Breanna Hawk, Jay Hawk, Shae Hawk, Dane Hawk, Ilan Hawk, Braylen Hawk, Liam Massey, Faith Massey, Elliot Hawk, Alijah Baum, Caleb Baum, Azreal McElwain and Muse McElwain.

Arrangements are under the direction of Owens & Brumley Funeral Home in Burkburnett The family requests that memorial donations be made in Jack's name to Hospice of Wichita Falls, or Central Baptist Church in Burkburnett.
