He was predeceased by: his parents, Luther Middlebrook and Bessie Middlebrook; and his brother Daryl "Dee" Middlebrook. He is survived by: his daughters, Lesha (David Penny) and Amber Middlebrook (Augie Meza); his grandchildren, Morgan, Kane, Crystal, Desiree and James; his nieces and nephews, Rhett, Bridgette Middlebrook, Heather, Steven Byrnes, Holly and Philip Pessoa; and his great-nieces and great-nephews, Thomas, Hailey, Nathan, Bella, Bea, Hannah, Charlotte, Sebastian and Henry. He is also survived by many cousins and precious friends.
