He was predeceased by: his parents, Rickey Lynn Polk and Tonja Leann Polk (Rhodes); and his brothers, Casey Polk and Ricky Polk. He is survived by: his wife Nicole Polk; his children, Brylen, Emerson, Teagan and Cheston; his brothers, Dusty Polk and Baley Polk (Brooke); his nieces and nephews, Landon, Avery, Preston, Harper, Kiairra, Everley, Presley, Madilyn, Bly, Beck and Micki Bassinger; his brothers-in-law, Russell Morganstean (Destiny), Travis and Taylor Harris; and his parents-in-law, Teresa and Chris Morganstean. He is also survived by aunts, uncles, and cousins.
