He was predeceased by: his parents, Adolf Koudelka and Lillie Koudelka; his wife Bette Koudelka; his sister Darlene O'Neal; and his son-in-law Ralph "Bucky" White.
He is survived by: his children, Danna (Warren), Beverly, Paige (Johnny), Ben, Jr. (Christina), Eric (Kathy), Barrett (Jennifer), Kenny and Karl (Jill); and his siblings, Evelyn Weisheit and Bernard Koudelka. He is also survived by numerous grandchildren and great-grandchildren.
