He was predeceased by: his father Donald A. Van Slyke; and his brother Scott A. Van Slyke. He is survived by: his mother Patricia Van Slyke; his wife Debi Van Slyke; his children, Tony Van Slyke, Patrick Miller (Elizabeth), Jeremy Miller (Kristen) and Jessica Mallett (Jason); his father-in-law David Peach; and his sisters, Pamela (Jim Turner), Lisa Speir, Lori (Mike Arnold) and Vicki (Steve Melton). He is also survived by 9 grandchildren, 1 great grandchild and numerous nieces and nephews.
