He was predeceased by: his parents, Joe Cook and Florence Pauline "Polly" Cook; and his wife Linda Kay Cook. He is survived by: his daughters, Teri Pate (Danny Johnson) and Carol Lowery (Matt); his grandchildren, Taylor Pate (Irwin Cortes), Max Lowery and Ella Lowery; and his great grandchildren, Rey and Noah Cortes.
