He is survived by: his wife Leila Cartlidge Lewis; the mothers of child, Zane Lewis and Angela Lewis Collins; his sons, Seth and Joshua Lewis; his grandchildren, Kayle and Chloe Lewis; his brother Jamie Lewis (Sherry); his sister-in-law Bennie Jo Lewis; and his nieces and nephews, Jason Lewis, Tammy Lewis Smith, Danny Denn, Brian Lewis, Sadie Kopecky, Brittany Lopez and Tiffany Odstrcil. He is also survived by Many aunts, uncles, cousins, great nieces and nephews, extended family, and friends.
