He was predeceased by: his daughter Beverly Cook; his grandson Corey Cook; and his brother Jerry Cook. He is survived by: his wife Dianne Cook of Axtell; his son Jay Cook (Valerie); his grandchildren, Randall, Jason, James, Raymond and Jessica; his great grandchildren, Brooke, Logan and Quin; and his brothers, Kenneth Cook (Anna) of Crawford, Mickey Cook (Loretta) of Waco, Leslie Cook (Kay) of Axtell and Paul Cook (Jimmie) of Axtell.
