He was predeceased by: his wife Sherry Mathis; his parents, Charlie Mathis and Imogene Mathis; and his sister Wanda Tamplain. He is survived by: his sister Nancy Duke; his stepchildren, Millissa Perkins of Longview and Jeremy Perkins (Janet) of Hawkins; his grandchildren, Andre, Ashlee Perkins of Longview, Cassie Buchannan of Harmony and Jeffery Perkins of Hawkins; his great grandchildren, Kameron, Adelaide Perkins and Kerrigan Buchanan; his nephews, Trevor Tamplain, Avery and Jesse Duke; and his brother-in-law Don Tamplain. He is also survived by great nieces, great nephews, cousins and friends.
