He is survived by: his mother Martha Lewis Sullivan of Blackjack; his wife Erica Wade; his daughter Trinity; his sister Amy Mazingo (Jeremy); his nieces and nephews, Madison (Jared), Morgyn, Molly, Harper and Devyn; his siblings, Robb Sullivan (Tracey), Texie Kersey, Teri Gibson (Mark), Tyann Foster (Mark) and Tracie Newcomb (Lewie); his brother-in-law Vincent Wade; his mother-in-law Debora Holden (Andrew); his aunts, Jolene Taylor, Joanna Lewis and Sheryl Sullivan; and his uncle Joe Mark Lewis. He is also survived by nieces, nephews, and cousins.
