She was predeceased by: her parents, Martha Louise Mangum (Pedigo) and David Mangum. She is survived by: her husband David Forman; her brothers, Kenneth Mangum and David Mangum; her aunt Janice Greer; and her uncle Danny Pedigo. She is also survived by numerous nieces, nephews.
