He was predeceased by: his parents, Evelyn Grace Kooistra (Ostrom) and Ernest Kooistra; his grandson Jack Hodge; and his sister-in-law Karen Kooistra. He is survived by: his wife Janine Kooistra (Padgett) of Fort Worth; his son Jamey Hodge (Cathy) of Arlington; his granddaughters, Katie Bodzinski (Eric) and Caroline Dunnett (David); his great grandchildren, Olivia and George; his brother Tommy Kooistra; and his niece Kristin.
