He was predeceased by: his mother Kimberly Kay Reese Hayworth. He is survived by: his father Randy Hayworth; his sisters, Dana Halverson (Jeremy Newell) and Sarah Boswell (Trenton); his close friend Jenny Davis; and his nieces and nephews, Matthew Beck, David Goldberg, Tyler, Carter, Aiden, Owen Boswell, Bailey Goldberg, Jesseigh, JacieKaye and Jillianne Boswell. He is also survived by numerous aunts, uncles and cousins.
