He is survived by: his brother John Hazelwood (Patti); his nephew Raymond Hazelwood; his fiancée Samantha; his brother Keith Gregory Hazelwood (Margaret); his nephews, Keith Jr., Matthew and Lucas Hazelwood; his sisters, Lydia Kay Hazelwood and Phyllis Lawson; his niece Elizabeth Suarez; and his nephew Buck Lawson.
