He was predeceased by: his grandmother Carolyn Cookston. He is survived by: his children, Hunter, Braden and Desiree; his parents, Randy Cookston (Angela) and Nina Henry; his siblings, Heath (Terri Lynn) and Haven Cookston; his grandparents, Ronnie Cookston, Bill and Brenda Parker; his nieces and nephews, John, Jayce, Finley and Shaylee; his uncle Derek; and his aunts, Cindy and Bobbi Fay.

In lieu of flowers donations can be made to Griffith Creek Community Church.
