He was predeceased by: his parents, Glenn R. Livesay and Mary Anne Underwood; and his brothers, Ernie and Darrell. He is survived by: his wife Kathy; his sons, Jason, Phatty, Phat Boy, Jeremy and Jelly Bean; and his sisters-in-law, Brenda Livesay and Mary Livesay. He is also survived by several nieces and nephews.
