He is survived by: his wife Betty Adams Forkum; his children, Mike Forkum of White Bluff, Mark Forkum (Sasha) of Phoenix, AZ, Steve Forkum of Bolivar and Laura Forkum Crowe (Donald) of Memphis; and his sister Clara Settles. He is also survived by 8 grandchildren and 1 great grandchild.

In lieu of flowers, memorials may be made to Camp Leatherwood or Church of Christ Disaster Relief Fund.
