He was predeceased by: his parents, William M. Forman and Mable Edwards Forman; and his wife Joyce Bowling Forman. He is survived by: his daughters, Cindy Forman McKelvey of Huntsville, AL, Tinny Jo Phillips of White Bluff, TN and Barbara Jean Byard of Smyrna, TN; and his brother George "Bill" Lewis Forman of Odenville, AL. He is also survived by 4 grandchildren; 8 great grandchildren.
