He was predeceased by: his parents, Perry James Cook and Marie Henson Cook; and his great grandchild Poppito. He is survived by: his children, Douglas W. Cook (Greysy) of Jackson, Dennis W. Cook (Michelle) of Jackson, Bradley Cook (Cindi) of Union City and Debra Cook Jenkins of Union City; and his sister Paula Tidwell (Duane) of Trimble, Tennessee.. He is also survived by his fourteen grandchildren.
