He was predeceased by: his daughter Tammy; and his siblings, Gene, George, Mary and Nadine.
He is survived by: his wife Barbara Maples (Helms); his children, Allen Surgenor (Sandy), Tesia Glenn (Mike) and Misty Williams (Tim); and his siblings, Loretta Maples and James Maples. He is also survived by several nieces and nephews also survive and several special friends.
