He was predeceased by: his siblings, Estel Middleton, Ernie Middleton, Jewel Middleton, Jess Hayden Middleton, Ruth L. Middleton Hale and Dorothy Lee Middleton Bailey.
He is survived by: his brothers, Rev. Dennis Middleton (Louisville), Rev. Carl Middleton (Louisville) and Fred Middleton (Rebecca).
