He was predeceased by: his parents, Titus Page Sr. and Della Yates Sullivan; his wife Julia Kerr Sullivan; his former wife Mary Elizabeth Sullivan; his son Jerry Wayne Sullivan; his siblings, Titus Page "T. P." Sullivan Jr., Chester Sullivan, Jim Sullivan, Titus Lee Sullivan, Beecher Sullivan, Daniel Sullivan, Leonard Sullivan, Larry Sullivan, Ira Lee Sullivan, Nathan Sullivan, Vida Madewell, Lola Mitchell, Clata Ray Mooneyham, Kathleen Simmons and Betty Sullivan; and his nephew Bobby Simmons.
He is survived by: his siblings, Wade Sullivan (Brenda), Marlin McCoy (Gary) and Ella Jane Knowles (Paul); and his nephews, Ricky Simmons (Patty) and Ronnie Simmons (Ann). He is also survived by Many nieces, nephews, cousins.
