He was predeceased by: his parents, Hershel Blaylock, Sr. and Linda Davis Blaylock; his wife Amy Blaylock; and his sister Sandra Blaylock Paul. He is survived by: his brother Hershel Blaylock Jr. (Charlene); and his nieces, Racheal Blaylock, Hannah Hodge, Abbi Usie, Dana Good and Cara Usrey.
