He was predeceased by: his parents, Arlo Cook and Shirley Walker Cook; and his grandparents, Cecil, Maggie Walker, Mervin and Nina Cook.
He is survived by: his daughter Aja Cook; his grandchildren, Woods Walker of Rickman and Arie Smith of Rickman; his siblings, Jeff Cook (Linda) of Smithville, Johnny Cook (Rachel) of Smithville, Gerwin Cook of Arizona, Deborah Young (Danny) and Crystal Petty of Smithville; and his girlfriend Tiffany Hendrixson of Smithville. He is also survived by several nieces and nephews.
