He is survived by: his mother Thelma Smith Cook; his wife Frances Sue Ervin Cook; his sons, Dustin Cook of Shelbyville and Matthew Cook (Julie) of Shelbyville; his grandchildren, Jaden Cook, Alexander Puckett, Makenzie Cook and Joseph Cook; his brothers, Roger Cook of Shelbyville and Darron Cook (Dawn) of Pikeville; and his nephew D. J. Cook.
Visitation was held on Wednesday, December 18th 2024 from 11:00 AM to 1:00 PM at the Chapel of Doak-Howell Funeral Home (739 N Main St, Shelbyville, TN 37160). A funeral service was held on Wednesday, December 18th 2024 at 1:00 PM at the same location.
