He was predeceased by: his parents, Owen Thomas and Ida June Cook (Johnson); and his brothers, Garry Kendall Cook and Grover Cleveland Cook. He is survived by: his nephews, Quincy Cook of Sharps Chapel and James Ryan Cook of Germany; and his niece Bolden of Tazewell. He is also survived by family members and friends.
