He was predeceased by: his father Howard Lewis Sr.; and his brothers, Harold David Lewis and Howard Kash Jr.. He is survived by: his mother Betty Kash (Phillips); his stepson Michael Ems of Sevierville; his stepgranddaughter Audrey Marie Ems; his nieces and nephews, Howard Kash III, Joey Isaacs, Noelle Easton and Selena Isaacs; his aunts, Wanda Goble and Barbara Phillips; and his uncle Tommy Phillips. He is also survived by other relatives and friends.
