He is survived by: his wife Jessica Mayfield of Crump, Tennessee; his daughters, Patience Mayfield of Crump, Tennessee, Love-Lee Mayfield of Crump, Tennessee, Heavenlee Mayfield of Crump, Tennessee and Liberty Mayfield of Crump, Tennessee; his parents, Berl Mayfield (Lisa) of Franklin, Tennessee and Susan Baldwin Mayfield of Murfreesboro, Tennessee; his sisters, Michelle Pewitt (Hank) of Saltillo, Natalie Anderson (Chris) of Murfreesboro and Dorothy Mercado (Gabriel) of Franklin, Tennessee; and his children, Jim, Virginia Mayfield, Don and Tennie Baldwin. He is also survived by several nieces and nephews.
