He was predeceased by: his parents, Wayne Lewis and Margaret Eustice; and his brothers, Kit Eustice and Gary Wayne Lewis. He is survived by: his daughter Michelle Ann Lewis; his spouse Gail Lewis; his children, Angela Chambers, David Lewis (Pam), Paula Lewis Smith and Christy Painter (Josh); his grandchildren, Becky Higdon, Timmy May, Brandon Chambers, Christopher, Caitlynn, Caleb, Candyce Lewis, Hope Lewis, Ashton Smith, Aidan Smith, Addie, Bentley and Olivia Kate Painter; his great grandchildren, Cameron May, Landon Chambers, Silas Ferguson and Samson Ferguson; and his siblings, Pete Eustice (Penny), Kim Eustice (Susan), Marsha Potts (Norman), Sharon Scealf (Jim) and Kenneth Conaway. He is also survived by several nieces and nephews.
