He was predeceased by: his father Dr. Thurman L. Smith; his wife Jean; and his brother Dr. L. Dennis Smith. He is survived by: his children, Sean Smith and Samantha Cooper; his siblings, Brenda Gaskins of Westfield, IN, Leslie Mohlke and Jennie Otterman; and his grandchildren, Danielle Smith, Taylor Smith and Kaylee Smith. He is also survived by many nieces and nephews.
