He was predeceased by: his parents, Clyde Collins and Wanda Collins. He is survived by: his daughter Chelsey Cook (Dustin); his stepmother Bernice Collins; his grandchild Grayson; and his sister Margaret Miller (Mike). He is also survived by several friends, cousins, and other family members.
