He is survived by: his wife Carol Ann Cook; his children, Jennifer (Kyle Gardner), Johnathan (Kim Cook) and Daniel Cook; his grandchildren, Erika Seiber, Taylor Cook, Austin Cook, Anna Seiber, Gracie Cook, Allie Gardner and Masie Cook; and his brothers, Larry (Katie Cook), David Cook, Kenny (Gail Cook) and Dwayne Cook. He is also survived by numerous nieces and nephews.
