He is survived by: his sons, Shane Forbis (Sheri) of Franklin, KY and Brandon Forbis (Jade) of Portland, TN.; his sisters, Jo Ann Wheeler (Bobby) of Portland, TN and Teresa Mallory (Donnie); his former wife Teresa Forbis of Portland, TN.; his grandchildren, Jason Forbis, Aiden Radican, Sue Gann, Samantha Gann and Shana Forbis; and his great grandchildren, Levi, Jacob, Arya and Rylee.
