He is survived by: his wife Dawn Creekmore; his sons, Brandon Creekmore (Amanda) of Portland, TN. and Scott Creekmore of Portland, TN.; his siblings, Jimmy Creekmore of Hendersonville, TN, Bobby Creekmore of Portland, TN. and Paula Gram of Nashville, TN; and his grandchildren, Easton, Alexis, Bailei, Connor, Zach and Riley Creekmore.
The family has also requested in lieu of flowers donations be made to the Humane Society of Sumner County, PO Box 2412 Hendersonville, TN 37077.
