He was predeceased by: his mother Dorothy Perry Aldridge. He is survived by: his father Jerry Lee Phillips; his wife Shalena McClister Phillips; his children, Hailey Phillips, Ricky Caldwell, Bradley Michael Meridieth and Caleb Madlena (Julie); his siblings, Bruce Phillips (Susie), Mary Phillips, Shelia Whitman, Tammy and Teresa Campbell; and his grandchildren, Everleigh and Houston. He is also survived by several nieces and nephews.

The family requests donations to be made to help with funeral expenses.
