He was predeceased by: his parents, Jack, Sr. Mayfield and Ruby Mayfield; and his brother Daniel Mayfield. He is survived by: his siblings, Richard Mayfield of Newport and Wilma Van Stratum (Calvin P. J.) of Newport; and his nephews, Joey, Jacob, Blake Van Stratum and Timmy Henderson (Sissy).
