He was predeceased by: his parents, Dan Lewis and Maxie Hannon Lewis; and his brother Kenneth Lewis. He is survived by: his wife Betty Jo (Barnes); his children, Jimmy Lewis (Jennifer), Johnny Lewis (Sandy), Janet Thompson (Tim), Jackie Campbell (Teddy) and Jerry Lewis (Lynn); and his grandchildren, Shannon Thompson, Shelley Campbell, Sheena Santana, Matthew Lewis and Elizabeth Watt. He is also survived by nine great-grandchildren.
In lieu of flowers, the family requests donations be made to the Alzheimer's Association or the charity or church of your choice.
