He was predeceased by: his parents, Jesse Cook and Alma Pauline Cook (Castleow); his brother James Cook; and his stepdaughters, Debbie and Terri Somers.
He is survived by: his wife Mary Cook; his daughter Angel Sullivan (Marty); his stepson Donnie Somers; his grandsons, Codye Sullivan (Kati) and Caleb Sullivan; his stepgranddaughters, Lisa Schoot (Rock) and Melissa Storie (Noah Krantz); his siblings, Richard Cook, Joyce Lee (Don), Carolyn Ervin (James) and Shirley Kendall; and his pet Domino. He is also survived by many great-grandchildren.
