He was predeceased by: his parents, Willie Cook and Annie Lou Cook; his brothers, James Cook, Charlie Cook and Lee Cook; and his son Jeff Cook.
He is survived by: his wife Carolyne Cook; his children, Debbie Cook, Keith Cook (Tracey), Pam Teas (Bill) and LuAnn Goodman; his stepchildren, Larry York (Joy), Chuck York (Cathy) and Jennifer Armstrong; his grandchildren, Dillon Cook, Kane Cook, Blake Goodman (Brittany), Gavin Teas, Jacob York (Courtney), Cayleigh Pazych (Nick) and Allison Armstrong; his great grandchildren, Bonnie Rae York and Easton Goodman; his brother Steve; and his sister-in-law Chrissy Spradling.
In lieu of flowers contributions may be made in Al's memory to Alive Hospice or St. Jude Children's Research Hospital.
