She was predeceased by: her parents, Lyle Robert Ratliff and Emily Matilda Wininger. She is survived by: her daughter Lisha Ann Collingwood (Spencer); her stepgrandchildren, Daniel and Amelie; her cousin Lois Seal (Windel); her niece Susan Hough; and her nephews, David and George Buffington (Katie).
In lieu of flowers Phyllis requested that memorial contributions be made to The Bridge Home No Kill Animal Rescue in her name.
