He is survived by: his children, Allison, Emily Polk, Thomas Polk Jr. and Joshua Polk; his grandchild Anaya Ahmed; his former wife Nina Polk; his sister Jennifer Lee Sapp; his close friend Dom; and also Alaaeldin.
