He was predeceased by: his wife Mary Catherine Cook; his parents, Guy Cook and Ella Jane Cook; his children, Timothy Dwayne Cook, Kenneth Cook and Ella Jane Cook; and his siblings, Mary Lou Richardson and Frank Cook.
He is survived by: his children, Jerry Lynn Cook (Kathy), Robin Jeanette Craig (Tommy White), Robert Matthew Cook (Alisha Arms) and Catherine Michelle Cook; and his sister Della Jane Rainbolt. He is also survived by several grandchildren and great-grandchildren; and several nieces and nephews.
