He was predeceased by: his parents, Jess Cook and Harriet Cook (Cozart); and his siblings, Clarence "Buddy", Howard Cook, Dorothy DeSardi, Minnie Henson, Annie Mullins, Margaret Lovell and Betty Chapman.
He is survived by: his siblings, Jess, Jr. (Mary), Johnny (Patricia), Lynn Cook, Rosa Diaz, Mary Spencer and Shirley Maxey (Buster); and his sister-in-law Dolores Cook. He is also survived by many nieces, nephews, great nieces, and great nephews, and a host of family and friends.
